A spreading infection of the skin involving subcutaneous fat (compare with erysipelas (image 237), which involves upper dermis). Mainly caused by GABHS and S. aureus. Commonly affects legs of elderly (image 344). A special mutant variety is ‘flesh-eating’ GABHS, which causes localised destruction of tissue.

Treatment Penicillin for GABHS and di(flu)cloxacillin for S. aureus. Attend to underlying cause (e.g. skin ulcer).
